Survivor Benefit Plan

Our office is aware of various issues surrounding the Survivor Benefit Plan. Most recently, constituents cited correspondence issues between them and the Defense Finance and Accounting Service (DFAS). We are familiar with the Certificate of Eligibility requirement that Annuitants must submit and the additional Child Annuitant’s School Certification for full-time students between 18-22 years old. We are looking into allegations that these documents are regularly returned to Annuitants for minor corrections, while submission and resubmission itself is a difficult process.

As we look into some of costumer service practices of DFAS and the processes that exist, we are interested in hearing from our constituents regarding your personal experiences with the claims process. Please submit the requested information below so we can continue to address these issues with DFAS. More information on the Survivor Benefit Plan is located below.
